Suda doğum nedir?
Su ile tedavi uzun yıllardır kas gevşetici ve rahatlatıcı etkileri nedeni ile kullanılagelen bir alternatif tedavi yaklaşımıdır. Bu etkinin normal doğumlarda da kullanılabileceği fikri de oldukça eskilere dayanır. Dokümente edilen ilk su altı doğumu 1803 yılında Fransa'da yaşanmıştır. Ancak bu planlı bir doğum değidir. Uzun süre doğum eyleminde kalan ve biraz rahatlamak için sıcak su dolu bir küvete giren bir kadının doğumu bu esnada gerçekleşmiş ve bu tesadüf sonucu suda doğum yapan ilk kadın olarak tarihe geçmiştir.
1960'lı yıllara kadar suda doğum ile ilgili herhangi bir gelişme yaşanmazken bu tarihlerde ilk kez eski Sovyetler Birliği'nde Igor Charkovshy bu konuda denemelere başlamıştır. Onu 1978-1985 yılları arasında Fransa'da yaşayan Dr. Michel Odent izlemiş ve su altında pekçok doğumun gerçekleşmesinde yardımcı olmuştur.
Suda doğum uygulamaları daha sonraları bir ara güncellik kazansa da belirli bölgeler dışında hiçbir zaman popülarite kazanamamıştır. Günümüzde eski Sovyet Cumhuriyetleri, İngiltere ve Fransa'nın bir kısmı ile Amerika Birleşik Devletlerinde sınırlı sayıda klinikte uygulanmaktadır.
Suda doğum yaptıran ve bu uygulamayı savunan kişiler ılık suyun sakinleştirici ve ağrı giderici etkileri olduğunu ve bu etkinin kadının kendisini rahat hissetmesine ve doğumun daha kolay geçmesine yardımcı olduğunu ileri sürmektedirler. Bu görüşler dışında suda doğumun su dışında doğuma üstün olduğunu gösteren hiçbir bilimsel veri yoktur.
Konuyla ilgili yapılan ve normal doğum ile suda doğumu karşılaştıran sistemik bir araştırmada yarar ya da istenmeyen etki açısından her iki doğum şeklinin birbirine karşı avantaj ya da dezavantajının olmadığı gösterilmiştir. 1994-1996 yılları arasında İngiltere'de gerçekleşen doğumların sadece %0.6'sı suda olmuş ve bu doğumların da %9'u evde gerçekleşmiştir. Bu doğumlarda bebek ölüm oranı binde 1.2'dir ve normal suda olmayan doğumdan çok farklı değildir.
Suda doğum tüm dünyada yaygınlık kazanmadığından konu ile ilgili bilimsel araştrıma ve makaleler de son derece sınırlı sayıdadır ve bunların büyük bir kısmı ebelik ile ilgili dergilerde yer almaktadır. Suda doğum klinikleri de genelde ebelerin görev yaptığı merkezler şeklindedir. Karşılaştırmalı inceleme yapılan araştırma sayısı ise yine çok kısıtlıdır ve eldeki veriler fikir birliğine varmak için yeterli değilidir. Konuyla ilgili çelişkili bilgiler mevcuttur.
Bazı çalışmalarda suda doğum sırasında annede daha fazla sayıda ve daha ciddi doğum kanalı yırtıkları ortaya çıktığı ileri sürülürken bunun tam tersini bildiren çalışmalarda vardır. Benzer şekilde suda doğum ile normal doğum karşılaştırıldığında doğum eyleminin süresi, ağrıkesici gereksinimi gibi parametreler açısından da birbiri ile çelişen bilgiler yapılan az sayıdaki araştırmalardan elde edilmiştir.
Suda doğumu savunanların hipotezi ılık suyun kasları gevşeteceği ve zihinsel rahatlık sağlayacağı ve bu sayede plasentaya giden kan akımının artarak daha az ağrılı ve daha kısa bir doğum süreci yaşanacağıdır. Ancak burada suyun sıcaklığı önem kazanmaktadır.Su için ideal sıcaklık 37 derecedir. Suyun daha sıcak olması durumunda anne adayının kan dolaşımında değişim olabilir ve ani tansiyon düşüklüğü ile plasentaya giden kan akımlarında azalmalar yaşanabilir bu da hem anne adayını hem de bebeği gereksiz risk altına sokabilir. Ayrıca suda uzun süre kalınması durumunda anne adayında terlemeye bağlı sıvı kaybı görülebilir. Öte yandan doğum eylemi sırasında anne adayı su içindeyken bebeği kardiyotokograf ile monitörize etmek oldukça güçtür. Bunun için özel monitör cihazları gereklidir. Doğum eylemi monitörüze edilmediğinde bebeğin kalp seslerinde yaşanabilecek düşmeler fark edilemeyeceğinden oksijensiz kalması riski söz konusu olabilir.
Suda doğumla ilgili bir başka risk de enfeksiyon olasılığındaki artıştır. Doğum eylemi sırasında suya karışan kan ve dışkı hem anne hem de bebek için risk yaratır. Her ne kadar sudaki anneye ait dışkı su dışına alınsa da su hiçbir zaman temiz olmamaktadır.
Suda doğum sırasında karşılaşılan ve önceden kestirilemeyen bir başka risk de kordon kopmasıdır. Özellikle bebeğin göbek kordonunun kısa olması durumunda aniden su yüzüne çıkan bebeğin kordonu kopabilir ve bebek kan kaybedebilir. Yapılan bir çalışmada suda doğum sonrası bebeklerin %14'ünün kordon kopması nedeni ile yoğun bakıma alındığı ve hatta bir bebeğe kan verilmesi gerektiği saptanmıştır.
Doğumun yapılacağı havuzun fazla derin tutulmaması ya da bebeğin tamamen doğana kadar yukarı çekilmemesi bu riski azaltabilir.
Solunum açısından bakıldığında ise suda doğum bebeğin boğulma ya da su yutma riskini arttırmamaktadır.
Görüldüğü üzere suda doğum normal doğuma herhangi bir üstülük sağlamamaktadır. Kaldı ki evrim süreci içerisinde suda yaşayan pekçok canlı üremek için karaya gelmeyi tercih etmekte, karada yaşayan hiçbir canlı ise bu amaçla suya gitmemektedir.
Ülkemizde suda doğum ile ilgili tecribesi olan hekim sayısı neredeyse hiç yoktur. Dünyada yaygın uygulama alanı bulamamış bu yöntemin ülkemiz de de popülerlik kazanmasını uzak bir olasılık olarak görmekteyim. Ayrıca sağlık mevzuatında konu ile ilgili herhangi bir düzenleme bulunmaması nedeni ile görülebilecek olumsuzluklar karşısında yasal prosedürün de bilinmemesi nedeni ile pekçok jinekolog bu doğum şeklini uygulamaya yanaşmayacaktır.
